Jordyn Nelson
Program Manager
Jordyn Nelson joined Plymouth County Outreach in July 2023. Following the loss of her brother to a fatal overdose in September 2020, Jordyn developed a passion for deflection efforts, substance use prevention, recovery support, and community outreach, leading her to transition into this field.
In her role at Plymouth County Outreach, Jordyn conducts county-wide trainings for law enforcement, manages day-to-day functions. Jordyn serves as the Chair of the Plymouth County Situation Tables. In this role, she collaborates with community partners, public safety agencies, and service providers to identify individuals and families experiencing elevated risk across multiple areas of need. Through these partnerships, the Situation Tables provide timely connections to community-based services aimed at reducing harm and improving outcomes throughout Plymouth County.
Jordyn is certified in Mental Health First Aid through the National Council for Mental Wellbeing and is a certified facilitator for the Adapting Nurturing Fathers program. She completed the Recovery Coach Academy through the Connecticut Community for Addiction Recovery (CCAR) and has participated in several trainings through the Boston Medical Center Grayken Center for Addiction. In 2025, Jordyn presented at the National RX and Illicit Drug Summit, sharing the work of the Plymouth County Situation Tables. She has also attended numerous national conferences focused on substance use, public health, community-based response efforts, and law enforcement collaboration. These include the COSSUP National Forum, the Police Assisted Addiction and Recovery Initiative (PAARI) National Summit and the Police, Treatment and Community Collaborative (PTACC) Summit. She remains committed to continuing her education to expand her knowledge in the field.
